What Is Liposuction?

Liposuction is one of the most popular body contouring plastic surgery procedures in Nashville and nationwide. During liposuction surgery, Dr. MaDan creates small incisions in the skin to access the stubborn fat. These incisions are typically the size of your small fingernail–about 1-2 mm in length. The tumescent solution, which is a mixture of IV saline, lidocaine, and epinephrine, is infiltrated throughout the treatment area for pain control and to minimize bleeding. A hollow suction tube called a cannula is inserted through the small incisions to remove the fat and fluid.

Preparing for Liposuction

Before your Nashville liposuction surgery, it’s essential to be in good overall health. While Dr. MaDan will customize liposuction treatments for each patient, here is some general information to help you prepare.

• Stick to a consistent exercise program, and eat a healthy, well-rounded diet with plenty of protein to help you maintain your stable weight and avoid weight fluctuations.

• Visit your primary physician for an examination and appropriate testing to ensure you’re medically fit for surgery. For any person aged 50 and over, we require routine labs, such as CBC, BMP/CMP, and EKG.

• Stop taking medications, supplements, or vitamins that increase bleeding or bruising risks for at least 2 weeks before and after surgery. For example, avoid fish oil, garlic, ginseng, vitamin E, and ibuprofen or other anti-inflammatory medications.

• If you currently smoke, vape, or use nicotine products, you should quit. As smoking is detrimental to your overall health and recovery, Dr. MaDan requires that patients stop smoking at least 6 weeks before and after surgery. A urine nicotine test will be performed on the morning of your procedure.

• Prepare your home and allow time in your schedule for recovery. Take adequate time off work, complete necessary chores and activities before surgery, and create a comfortable place to recover. It’s also helpful to freeze meals or set up a food delivery service. All necessary medications, supplies, and garments will be provided at your pre-operative appointment.

• Arrange for assistance. First, you will need a responsible adult to drive you home after your procedure and stay with you for at least 24 hours. You may also need help with household management or childcare for a few weeks following surgery.

Overview of Nashville Liposuction under General Anesthesia

On the night before surgery, don’t eat or drink anything after midnight. Take necessary medications with small sips of water. Avoid wearing makeup, lotions, or deodorants, and remove all jewelry. Wear comfortable, loose clothing that is easy to put on after your procedure.

Review your paperwork to ensure your surgery location and arrival time. At check-in, you may need to provide identification and emergency contact information.

On the day of your procedure, you’ll meet with Dr. MaDan and your anesthesia provider to review the details of your surgery and answer any final questions. Dr. MaDan will be with you the entire time while you are put to sleep.

Dr. MaDan will begin your liposuction procedure using the appropriate technique based on your anatomy and goals as discussed during your consultation. She will create very small incisions in areas that are easily concealed. Then, Dr. MaDan will add tumescent fluid (IV saline, lidocaine, epinephrine) to the treatment area. Afterward, she will insert a small suction tube called a cannula to remove the fluid and the fat.

Often, liposuction incisions are very small and do not require sutures. Dr. MaDan will cover the incisions with steri-strips that will remain in place until your first post-operative appointment.

Following your liposuction procedure, you will be dressed in a compression garment to minimize swelling and support the new contours.

The procedure may last 1 to 4 hours depending on the number of areas being treated and combination skin removal/contouring procedures performed at the same time.

In-Office Liposuction Overview

Liposuction is commonly performed in our office surgical suite under local anesthesia. For in-office liposuction under local anesthesia, you can eat and drink normally on the morning of surgery. If you’re having a combination surgery under general anesthesia, avoid eating or drinking anything after midnight before your surgery. Necessary medications can be taken with a small sip of water. Avoid wearing makeup, lotions, deodorants, and be sure to remove all jewelry. Wear comfortable, loose-fitting clothing that is easy to put on after your procedure.

Please review your paperwork carefully to confirm your surgery location and arrival time. At check-in, you may need to provide identification and emergency contact information. Depending on the extent of the procedure, in-office liposuction typically takes one to two hours.

For in-office procedures, a local anesthetic will be injected to numb the area and control pain. In some cases, oral sedation may also be provided to help you relax.

Dr. MaDan will choose the appropriate liposuction technique based on your anatomy and aesthetic goals. Small incisions are made, and a thin tube called a cannula is inserted to remove excess fat and sculpt the area to your desired contours.

After the procedure, small incisions will be closed with stitches and covered with dressings. Dr. MaDan will provide you with detailed aftercare instructions to help ensure a smooth recovery and optimal results.

Nashville Liposuction Recovery

• Liposuction is an outpatient procedure, meaning you will go home the same day after surgery. Arrange for someone to drive you home from your procedure and help you with household tasks, including childcare or pet care, for a few days. Avoid sleeping on your nicest bedding, as drainage from the liposuction site(s) is normal for the first 24 - 48 hours.

• For the first few days following your liposuction procedure, you will experience mild to moderate discomfort and feel tender or sore. Some patients feel like they have completed a tough, strenuous workout. You should expect mild bruising and swelling that will gradually subside in a few weeks. It’s important to avoid pressure on the treated area for a few days. Mild but tolerable pain is expected after the procedure and should be managed with the medication plan provided to you by Dr. MaDan at your pre-operative appointment.

• Before you are awakened from surgery, you are fitted into a surgical compression garment. The garment is an essential component to your recovery. It functions to minimize swelling and support the tissues as they are healing. The compression garment should be worn continuously except when showering.

• To protect your incisions and allow tissues to heal properly, avoid strenuous activities like exercise and heavy lifting—more than 10 pounds—for at least 2 weeks. Patients can resume light exercises like walking or riding a stationary bike after 2 weeks.

• Liposuction recovery depends upon the extent of the procedure and the body’s healing abilities. Most patients need 5 to 7 days for recovery and feel well enough to resume working at that time. Essentially, when you no longer need pain medications and can abide by Dr. MaDan’s restrictions, you can return to work.

How Soon Will I See My Nashville Liposuction Results?

Following liposuction procedures, patients often see immediate enhancements to their overall shape. However, it’s important to be prepared for swelling that may obscure the final outcome. Initial swelling is most severe in the first few days after surgery but greatly resolves within 2 weeks. Residual swelling may persist for another 1 to 2 months, with full results visible within 3+ months. The skin tightening effect is gradual and usually requires 3+ months for the final outcome. If additional energy like Renuvion is used with your liposuction, the resultant skin tightening and neocollagenesis requires 12 months for its full effect.

While liposuction creates very small scars, the scarring is most pronounced in the first few months as the incisions heal, forming a pinkish scar. It may take a year or more for the scar to mature and match your skin tone. The scar will never disappear entirely. Dr. MaDan will provide you with her medical-grade scar therapy regimen to optimize the appearance of your scars.

Lymphatic massage can be instrumental in your recovery after liposuction. Typically your first visit is within 3 to 5 days following your surgery. Ensure your lymphatic massage therapist has experience with post-operative liposuction treatments.

Nashville Liposuction FAQs

For at least 2 weeks after your liposuction procedure, avoid intense activities and heavy lifting of more than 10 lbs. These restrictions help to minimize bleeding risks, which can occur in the 2-week window after surgery if you are not compliant. Typically, it is acceptable to shower 48 to 72 hours after surgery. However, avoid swimming, soaking, or submerging your incisions for at least 2 weeks after your procedure.

While liposuction cannot stop gravity, aging, or genetics, results can be long-lasting by maintaining a stable weight through a nutritious diet and routine exercise. Liposuction permanently removes fat cells; however, it does not remove all fat cells from any treated area. If a person gains weight after liposuction, their fat cells do not multiply or increase in quantity. Instead, the remaining fat cells get larger, which may negatively affect their liposuction result. It is ideal to maintain your weight within 5 % of your weight at the time of surgery for long-lasting results.

Liposuction performed in one area does not cause fat growth in a different area. For example, liposuction performed on the abdomen does not cause fat growth in the arms. The fat cells removed during liposuction are permanently removed. Reducing fat cells in one area subsequently changes your body’s overall fat cell proportion/distribution. It is impossible to remove every single fat cell from a treatment area.

The remaining fat cells will respond to weight gain like any other fat cells—they will get larger (hypertrophy). With fewer fat cells in the area treated with liposuction, the untreated areas may appear larger, giving the illusion of fat growth in untreated areas.

Pain after liposuction varies for each individual. Liposuction in multiple areas at one time may cause a more painful recovery. Some patients describe the pain as like soreness after a tough workout. Some areas, like the inner thighs, are usually more sensitive than others.

Your post-procedure compression garment is essential to minimize swelling and provide support to the tissues while they are healing. Peak swelling usually occurs around 48 to 72 hours after surgery.

Medications for pain, muscle spasms, and inflammation are provided for you and can be very beneficial during your recovery. Bruising is also normal and will resolve with time. Homeopathic remedies, such as arnica and bromelain supplements, can be beneficial.

Sleeping positions after liposuction vary depending on the treatment area and patient comfort. For at least 6 weeks after surgery, you will be sleeping in your compression garment. The first few nights after surgery, it’s recommended to sleep on your back with your head elevated.

It’s important to expect drainage from your small liposuction incisions for the first 24 to 48 hours after surgery, so Dr. MaDan doesn’t recommend sleeping on your nicest sheets.

The primary difference between liposuction and a tummy tuck is skin removal. An abdominoplasty, or tummy tuck, removes excess skin and fat; liposuction only removes fat beneath the skin. Patients with loose, excess skin are more likely candidates for abdominoplasty with liposuction rather than liposuction alone. The ideal candidate for liposuction has tight skin without significant stretch marks and a small area of stubborn fat. After liposuction, there is a slight element of skin tightening, approximately 5 to 10%, which is likely due to scar distribution secondary to the use of the cannula.

Liposuction is not for weight loss. In fact, the scale is unlikely to change after liposuction. The ideal candidate for liposuction is someone who is already at their ideal weight with an area of stubborn fat. Liposuction is used to sculpt and shape, not to decrease body weight or % body fat.

The quantity and distribution of your fat cells is determined by your genetics. You may be born with more fat cells in a certain area compared to someone else. You may see genetic traits in one side of your family that are similar to yours. With significant weight gain, the fat cells do not multiply or increase in quantity (hyperplasia). Instead, the fat cells enlarge (hypertrophy). Liposuction can alter the proportions of fat cell distribution. After an area is treated with liposuction, the fat cells are permanently removed. However, with weight gain, the fat cells left behind will enlarge (hypertrophy).

Fat cells do not migrate or travel to different areas of the body after liposuction. With weight gain, all of your fat cells will enlarge. The proportion and distribution of your fat cells after liposuction will be different meaning there are fewer fat cells in the treated area. Weight gain after liposuction may make the treated area appear to be unaffected. This is an illusion. The remaining fat cells in the treated area continue to enlarge, however there are fewer in quantity.

Subcutaneous abdominal fat is the layer of fat between the skin and abdominal muscles. Visceral fat is internal abdominal fat–fat around your organs. Visceral fat cannot be treated with liposuction. Visceral fat is decreased with regular exercise and a healthy diet. Subcutaneous abdominal fat can be addressed with liposuction. It will decrease the thickness of the fatty layer over the abdominal muscles.

Typically, abdominal muscle definition is seen when the overall body fat percentage is minimal. This varies by gender and with genetics. For women, a body fat percentage less than 15 – 20 is usually needed for visible six-pack muscles.  For men, the body fat percentage may need to be less than 10 to 12 percent. A strict diet is the best way to achieve visible abdominal definition.

There is no skin removed with liposuction. Instead, a 5 to 10% skin tightening effect can be seen about three months post-procedure. This is likely just scar distribution from the procedure. Someone with skin laxity that needs more than 5 to 10 % tightening may be a candidate for liposuction combined with energy (like Renuvion). Renuvion with liposuction can add an additional 25 to 40% skin tightening effect. Someone with excess skin that needs more tightening than Renuvion can provide may be a candidate for liposuction and skin removal.

After liposuction, you can help maintain your results by committing to a healthy lifestyle with a balanced diet and regular exercise. Your compression garment should be worn for a minimum of 6 weeks post-treatment. Most patients will have long-lasting results by maintaining their weight within 5% above or below their weight at the time of surgery. Limit sugary drinks, processed foods, and starchy carbs like sweets and chips. Drink plenty of water, limit alcohol, and avoid all nicotine products. Balance your diet by eating nutrient-dense vegetables, fruits, proteins, and fast-burning carbs.

Yes. Fat removed from liposuction can be processed (washed and purified) and transferred to another area of the body. This procedure is called fat transfer or fat grafting. Dr. MaDan offers fat transfer to areas like the face to provide permanent volume to areas that sag and deflate over time.

During your liposuction procedure, Dr. MaDan will make small incisions (~2-3 mm or the width of your small fingernail) to remove fat from the targeted area. These incisions will be placed in areas that can be easily concealed.  Initially, these incisions may be red but will fade and become less noticeable over time.  Dr. MaDan will skillfully place incisions in natural folds or creases to minimize visible marks.   • Avoid blood thinning medications or supplements, and adjust your prescribed medications per Dr. MaDan’s instructions
  • Avoid alcohol 48 hours before surgery 
  • Arrange for someone to drive you to and from surgery, stay with you for at least the first night, and help with household tasks, especially child or pet care 
  • Get required lab tests or medical evaluations (CBC, BMP/CMP and EKG for any one over the age of 50 years)

The day before your procedure, ensure you have all the necessary documents, get a good night’s sleep, and:

  • Do not eat or drink anything after midnight
  • Avoid applying lotions, deodorants, or creams the morning of surgery 
  • Remove all jewelry and piercings
  • Take out contact lenses 
  • Wear loose clothing that will be easy to put on over the compression garment

Candidates for liposuction are those who are in good general health and close to their target weight. These individuals do not smoke, vape, or use nicotine in any form.

Another important factor to consider is the quality of the skin. Suitable candidates should have good skin elasticity and minimal loose skin or cellulite, as these factors will adversely affect the results.
