Procedure Expectations

On your surgery day, you should arrive at the surgical center two hours before your forehead lift procedure. If Dr. MaDan prescribed an anti-anxiety medication to relax you, you should take it when you arrive. When you check in, you must show your identification and insurance documentation.

Depending on Dr. MaDan’s recommendations, an anesthesiologist will administer either a general anesthetic or IV sedation to help you remain relaxed and comfortable during your surgery. Local anesthetic will be placed throughout the forehead and scalp. When beginning your brow lift procedure, Dr. MaDan will make an incision based on the planned surgical technique.

Dr. MaDan will make small incisions within the hair baring scalp, just large enough to accommodate an endoscope. This small surgical device contains a tube-mounted camera and uses specific instruments to dissect and lift subdermal tissues and muscles. The camera gives Dr. MaDan visual access to the surgical area without creating an extensive scar.

Dr. MaDan will make an incision along the hairline. This allows her to directly view and access the areas requiring surgical intervention. This approach allows for hairline advancement and lifting of the forehead and brow to create a more youthful appearance.

After completing your procedure, Dr. MaDan will meticulously close your incisions in layers using a combination of absorbable sutures, removable sutures, and clips.

A brow lift is a cosmetic surgical procedure designed to raise the position of the eyebrows, reduce wrinkles, and create a more youthful, refreshed appearance. It targets heavy, sagging brows and the deep horizontal lines on the forehead and glabella (area between the eyes). By lifting the skin and tissues of the forehead, a brow lift can open up the eye area, making the face appear brighter and more approachable. 

An open brow lift can also improve the appearance of the forehead and hairline. The hairline incision (pretrichial) allows for simultaneous forehead shortening and hairline advancement.

A temporal brow lift requires an incision hidden in the hair-bearing scalp near the temples to lift and reposition the lateral brow.

An endoscopic brow lift uses incisions hidden in the hair-bearing scalp but may not be an option for patients with a receding hairline or excess forehead height. An endoscopic brow lift has the potential to elongate the forehead slightly. The small incisions allow for minimal access and require the use of a surgical camera (endoscope) for the procedure.  

There is no mathematical formula to determine the perfect brow position. A youthful, beautiful face should not be defined by brow height alone. With aging, the brow sags and becomes deflated, and the forehead develops horizontal and vertical lines. The heavy, sagging brow can alter facial expression, leading to an angry, tired appearance. The goal of a brow lift is to raise the position of the eyebrows and soften the horizontal and vertical lines.

In many cases, lifting the brow position also improves the appearance of the upper eyelids. If there is persistent skin excess after the brow is lifted, an eyelid lift can be performed at the same time. If the brow is already full but the eyelid feels heavy with redundant skin, an eyelid lift may be sufficient to restore a refreshed look.

The benefits of a brow lift can include a softer, refreshed appearance and a more youthful hairline. Lifting the brow can also improve the appearance of the upper eyelids.

At your consultation, you will receive a comprehensive facial evaluation from Dr. MaDan. She will also use the VISIA skin analysis system to provide you with quantifiable data about your skin health. Dr. MaDan will want to know your specific goals and priorities regarding facial aging. After assessing your face and neck as a whole, a targeted plan of action will be developed. Aging affects all tissues in all areas. An appropriate plan should address multiple types of tissues with a combination of different treatment modalities. 

About 2 weeks before your surgery date, you will have your preoperative appointment with Dr. MaDan. During this visit, you will receive your surgery appointment details (location, arrival time, etc), prescriptions, and post-operative instructions. Dr. MaDan will review the surgical plan and address your questions and concerns. Avoid any blood-thinning medications like fish oil, NSAIDs, etc, for 2 weeks before surgery. If you are on a GLP-1 inhibitor, your dose should be held one week before surgery. Do not eat or drink anything after midnight before surgery. Your preoperative medications should be taken with a small sip of water. Wear loose, comfortable clothing and remove all jewelry and piercings. Have your identification ready when you check in at the surgery center. At the surgery center, you will change into a gown and meet your surgical team. You will see Dr. MaDan and review your surgical plan. Once the surgery is completed, you will stay in recovery until you are ready to be discharged home.

A brow lift is an outpatient procedure performed under general anesthesia at the surgery center. After you are asleep, local anesthetic is placed on the scalp and forehead for pain control and to minimize bleeding. The incisions are made, and the forehead and brow tissues are lifted. The deeper tissues are often secured using an anchor. A meticulous, multi-layer closure is then performed, minimizing damage to the surrounding hair follicles. A protective dressing is usually applied.

The best way to conceal a scar in the hair-bearing scalp is to ensure hair regrowth around and through the scar. Multi-layer closure of these incisions allows the deeper tissues to be closed with absorbable sutures. Staples are less damaging to the surrounding hair follicles compared to sutures. Using staples for the final closure in the scalp minimizes the risk of alopecia.

An endoscopic brow lift will have small incisions hidden within the hair-bearing scalp. A hairline (pretrichial), open brow lift will have a thin scar at the junction of the hairline and forehead. Initially, the scar may appear more prominent. Over time, it will fade and become almost imperceptible.

After your surgery, your scalp and brow may have a compression wrap dressing in place. Typically, this can be removed 48 to 72 hours after your surgery. Your scalp and forehead may feel numb. It is normal to experience swelling and mild bruising around the forehead, brow, and eyes.

Your brow lift results will be seen almost immediately. It is important to remember that initial swelling and bruising are expected and will resolve over time. Final results are typically seen three to six months after surgery.

A brow lift should not change your facial expressions. The elevation and repositioning of the brow should make the entire brow appear lighter and refreshed. The use of a neuromodulator, like Botox, can alter natural facial expressions by inhibiting muscle movement. Makeup is acceptable to wear 48 hours after your brow lift. If you have an anterior hairline incision, makeup can be applied 48 hours after suture removal. Suture removal and hair-bearing scalp staple removal usually occur 5 to 7 days after surgery. Protecting the scar with SPF and minimizing sun exposure is essential to allow the scar to soften and fade.

For the first two weeks after your surgery, all intense activities (anything that would elevate your heart rate or blood pressure) should be avoided. After two weeks, the post-operative bleeding risk has significantly decreased. Light activities like walking can be resumed after two weeks. Keeping your head elevated will minimize swelling around the eyes and brow.
