What Is Eyelid Surgery?
The eyelids are often overlooked when it comes to anti-aging rejuvenation. As the most delicate facial skin, the eyelids are prone to the effects of gravitational changes and environmental exposure. Loss of elasticity allows the tissues to sag downward over the lash line, affecting both your appearance and vision. Lower eyelids can sag and look puffy, sometimes exposing the inside of the lower eyelid, often creating an exhausted appearance. Eyelid surgery, medically known as blepharoplasty, is a safe and effective procedure that can be performed under local or general anesthesia.
An upper eyelid lift can often be performed in office, under local anesthesia, or under general anesthesia at the surgery center. The incisions are meticulously concealed in the natural lid crease after the precise removal of excess skin and herniated fat. Lower eyelid surgery must be performed under general anesthesia. The upper and lower lid procedures require different anesthesia due to the size of the surgical field. The anatomy of the lower lid is more confined, having less space for surgical dissection and hemostasis (controlling bleeding). To ensure your comfort and safety, Dr. MaDan will only offer lower-lid blepharoplasty under general anesthesia at the accredited surgical center. During lower eyelid surgery, the incisions are typically hidden on the inside of the eyelid or along the lash line to remove excess tissues.
Nashville eyelid surgery helps to improve the appearance of the eyelids by:
- Removing loose, sagging eyelid skin that impairs vision and interrupts the natural contour of the upper eyelid
- Repositioning or removing fat that causes puffy-looking eyelids and undereye bags
- Tightening drooping lower eyelids that reveal the white below the iris
- Diminishing the appearance of excess skin, fine lines, and tear-trough deformity on the lower eyelids
- Reducing the weight of sagging lids that can cause eye fatigue
With blepharoplasty procedures at BEYOND Collective, Dr. Olivia MaDan strives to create a refreshed and revitalized facial appearance. Dr. MaDan often performs eyelid surgery with a brow lift to optimize upper facial rejuvenation, helping you reclaim your confidence.
To create a more refreshed, rested appearance, Dr. MaDan understands that small changes produce significant results in the eye area, including:
- Brighter, more alert eyes for an overall youthful appearance
- Decreased bags underneath the eyes
- Reduced eye puffiness
- Fewer wrinkles around the eye area
- Healthy, natural-looking results
Procedures for Enhanced Facial Harmony
Patients considering eyelid surgery are typically concerned about other signs of facial aging or their skin’s appearance. Often, optimal blepharoplasty results rely on other procedures to create true facial harmony. To achieve dramatic facial rejuvenation and youthful harmony, eyelid surgery can be combined with other procedures, including:
- Dermal Fillers
- Neurotoxins
- Skin resurfacing
Eyelid surgery addresses signs of aging to help maintain a youthful appearance, but the appearance of the skin is important to your results. Dr. MaDan may recommend other non-surgical med spa treatments to complement your blepharoplasty, enhancing your overall results.

Preparing for Eyelid Surgery
Before your Nashville eyelid surgery, it’s essential to be in good overall health. Here’s some information to help you prepare.
• Stick to a consistent exercise program, and eat a healthy, well-rounded diet with plenty of protein to help you maintain your stable weight and avoid weight fluctuations.
• Stay well-hydrated before and after surgery.
• Visit your primary physician for an examination and appropriate testing to ensure you’re medically fit for surgery. We require routine labs, such as CBC, BMP/CMP, and EKG for anyone aged 50 or older.
• Stop taking medications, supplements, or vitamins that increase bleeding or bruising risks for at least two weeks before and after surgery. For example, avoid fish oil, garlic, ginseng, vitamin E, and ibuprofen or other anti-inflammatory medications.
• If you smoke, vape, or use nicotine products, you should quit. As smoking is detrimental to your overall health and recovery, Dr. MaDan requires that patients stop smoking at least 6 weeks before and after surgery. A urine nicotine test will be performed on the morning of your procedure.
• Prepare your home and allow time in your schedule for recovery. Take adequate time off work, complete necessary chores and activities before surgery, and create a comfortable place to recover. It’s also helpful to freeze meals or set up a food delivery service. All necessary medications, supplies, and garments will be provided at your pre-operative appointment.
• Arrange for assistance. First, you will need a responsible adult to drive you home after your procedure and stay with you for at least 24 hours. You may also need help with household management or childcare for a few weeks following surgery.
Eyelid Lift Procedure Overview
On the night before surgery, don’t eat or drink anything after midnight; however, take necessary medications with small sips of water. Avoid wearing makeup, lotions, or deodorants, and remove all jewelry. Wear comfortable, loose clothing that is easy to put on after your procedure.
Review your paperwork to ensure your surgery location and arrival time. At check-in, you may need to provide identification and emergency contact information.
Eyelid surgery typically takes 1 to 2 hours, depending on the individual characteristics of the procedure.
On the day of your procedure, you’ll meet with Dr. MaDan and your anesthesia provider to review the details of your surgery and answer any final questions. Eyelid surgery may be performed under local or general anesthesia, depending on the characteristics of your procedure.
Dr. MaDan will create small, well-concealed incisions within the natural structure of your eyelids. Then, Dr. MaDan will remove or reposition fat, tighten muscles, and address excess skin for a more youthful-looking appearance.
After achieving the desired contours, Dr. MaDan will use the finest permanent sutures for the most meticulous closure possible. She will cover the incisions with steri-strips that will remain in place until your first post-operative appointment. Your eyes may be covered to protect your incisions. The suture material may be blue or black in color. The permanent sutures will be removed at your first post-operative appointment. Typically, permanent sutures are less reactive and cause less inflammation and scarring.

Nashville Eyelid Surgery Recovery
• Eyelid surgery in Nashville is an outpatient procedure, meaning you will go home the same day after surgery. Arrange for someone to drive you home from your procedure and help you with household tasks, including childcare or pet care, for a few days.
• After surgery, your eyelids will feel tight. Your eyes may also be covered to protect your sutures and allow tissues to heal properly. We will provide ointment to help keep your incisions soft and moist. To reduce swelling, rest with your head elevated and use cold compresses. You may also need eye drops to ensure your eyes don’t dry out. Dr. MaDan may prescribe pain medications or recommend over-the-counter medications to alleviate discomfort.
• Following blepharoplasty, you will experience swelling, bruising, and some discomfort. These effects will gradually subside over the next few weeks. You may also experience sensitivity to light, blurred vision, or frequent tearing.
• Depending on your occupation and the speed you desire to return to work, Dr. MaDan may allow you to return to your work schedule when you don’t require any pain medication and can abide by the restrictions. Most patients choose to rest at home for a few days following the procedure. It is important to note that normal post-operative swelling may affect your vision for the first week following surgery. It can be helpful to wear dark sunglasses when you can’t avoid sunlight. Avoid strenuous activities like exercise and heavy lifting for at least 4 to 6 weeks. Typically, patients can resume light exercises like walking or riding a stationary bike after 2 weeks.
How Soon Will I See My Eyelid Surgery Results?
Following eyelid surgery, patients usually experience mild discomfort, swelling, and bruising that typically lasts 5 to 7 days. The normal swelling and visible sutures may result in social downtime for the first week after surgery. Dr. MaDan will remove the permanent stitches at your first follow-up appointment within 7 days following surgery. These side effects will obscure the final outcome before gradually resolving to reveal youthful, lifted, and rested-looking eyes. Minor residual swelling may last up to 6 months.
Eyelid surgery incisions are typically placed in inconspicuous locations within your natural eyelid anatomy. Scarring is most pronounced in the first few months, but incisions will heal and form a pinkish scar. Dr. MaDan will provide you with her medical-grade scar therapy regimen to optimize its appearance.
Sun protection is crucial following your Nashville blepharoplasty. It’s important to avoid squinting, so wear dark sunglasses when sun exposure is unavoidable. With proper skin care and sun protection, your eyelid surgery results should be long-lasting.

Eyelid Surgery in Nashville FAQs
A blepharoplasty is a cosmetic surgical procedure to address excess skin and fat on the upper and lower eyelids. An ideal candidate is someone who has excess skin or fat on the upper or lower eyelids with a full, youthful brow in the appropriate position.
An upper blepharoplasty decreases the amount of skin between the lash line and eyebrow. An ellipse of skin is removed and the incision is hidden in the natural crease of the upper eyelid. Excess fat can also be addressed with conservative removal. A lower blepharoplasty functions to remove excess crepey skin, remove or redrape the excess orbital fat and address tear trough deformity.
You may be a candidate for upper eyelid surgery if there is excess upper eyelid skin or fullness that can make you appear puffy or tired. Some women are bothered by excess skin and creasing when applying eye makeup. More extreme excess eyelid skin can be seen when it interferes with peripheral vision or hangs over the eyelashes.
Your consultation should include a comprehensive face and neck evaluation with specific attention to brow position, lid mobility, eyelid laxity and position, as well as history of dry eye or LASIK surgery. Typically, the best results are achieved when the face is considered as a whole as multiple factors contribute to the chief complaint of heavy eyelids.
Before your blepharoplasty, you should avoid all medications that may increase bleeding and bruising. Some examples include NSAIDs like ibuprofen and fish oil. Homeopathic supplements like arnica and bromelain can help with healing and minimizing bruising. Use of contacts for vision correction should be avoided on the day of surgery and the following week. Glasses are preferred during your initial recovery.
The upper blepharoplasty can be performed awake, in-office or asleep at the surgery center. A lower blepharoplasty is always performed asleep at the surgery center. The upper eyelid surgery removes an ellipse of excess skin between the lash line and the brow. The upper eyelid surgery does not alter the brow position. A small amount of fat is usually removed from the inner aspect of the upper eyelid. The incision and resulting scar lies within the natural eyelid crease. It is typically closed with a permanent suture that is removed around 5 days post-procedure.
A lower blepharoplasty functions to address two main components: tear trough deformity (“under eye bags”) and excess skin. The tear trough deformity is corrected by reducing or repositioning the inferior orbital fat. This can be performed through a skin or inner eyelid incision. After the lower lid contour is improved, excess skin can be removed if necessary. If skin removal is performed, the incision and resulting scar is usually hidden within the lower lash line or natural crease. This incision is closed with permanent sutures that are removed around 5 days post-procedure.
Procedure duration depends on the surgical plan and steps required to achieve the desired results. In general, an upper blepharoplasty takes less than 1 hour. A lower blepharoplasty can take around 2 hours.
Any incision will leave a scar. The incisions and resulting scars from eyelid surgery are placed within natural creases ensuring they will be almost invisible. Typically, a scar initially appears red and will soften and fade over time. UV protection is essential to minimize the risk of pigmentation. Our Lumecca IPL (intense pulsed light) laser can be used to treat any eyelid scars with appropriate corneal/eye protection.
Mild bruising and swelling can be expected after eyelid surgery. The permanent stitches used for closure may be visible until they are removed at day 5 post-procedure. Swelling can cause blurry vision. Cool compresses on the eyes are extremely beneficial to minimize bruising and swelling. They should be applied for 20 minutes every hour while awake for the first few days after surgery. Keeping your head elevated will also minimize eyelid swelling. Lubricating drops or gel can be essential in preventing dry eye during the healing process.
Most patients will notice immediate results after their eyelid surgery. The final results will be masked by the expected post-procedure swelling. Mild bruising may last 1 – 2 weeks and final results are typically seen 12 weeks post-procedure. The scar will continue to improve over the following six to twelve months after surgery.
There is no surgery that can stop the effects of time, gravity, genetics and aging. The ideal duration of blepharoplasty results is at least 10 years. Environmental and lifestyle factors will contribute to the duration of your results. Maintaining a healthy lifestyle (no nicotine use, no UV exposure or tanning beds) and consistent use of medical-grade skin care is the best way to protect your investment.
The most significant risk associated with eyelid surgery is bleeding. Dr. MaDan will never leave the operating room if something is bleeding. However, bleeding can occur within two weeks post-surgery. The purpose of your post-surgical restrictions are to minimize the risk of bleeding. Post-surgical bleeding is a medical emergency. If left untreated, the bleeding can lead to a “retrobulbar hematoma” which can result in permanent vision loss.
Other uncommon risks can include infection, asymmetry, corneal abrasion, dry eye, and lid malposition.
Yes. A blepharoplasty can be easily combined with other facial procedures like a brow lift, face and neck lift for comprehensive facial rejuvenation. Eyelid surgery can be performed in conjunction with other breast or body contouring procedures. This allows the consolidation of your recovery and down time.
At your initial consultation, your surgeon should perform a comprehensive evaluation of your face and neck. Your surgeon should listen to your goals and concerns to ensure the procedure will deliver the desired results. You should leave the consultation feeling educated about the range of options (non-surgical and surgical) to address the brow and eyes. Review before and after photos and discuss complication management with your surgeon.
While eyelid surgery can eliminate heavy, excess skin, it is unlikely to change your prescriptive vision correction. Patients desiring LASIK should undergo the procedure a minimum of 6 to 12 months before any eyelid surgery.
A surgical blepharoplasty is the only procedure to remove excess eyelid skin. All other alternatives will simply improve the quality, texture and appearance of your eyelid skin.
During your initial recovery, eyelid manipulation should be avoided. It is recommended to wear glasses if needed for the first 1 to 2 weeks post-procedure. After the initial recovery, use of contact lenses is acceptable.
Return to normal activities can occur after 2 weeks post-procedure. Depending on your occupation, you may be able to return to work when you are not needing pain medication and able to abide by the lifting restrictions.
Mild discomfort is expected after eyelid surgery. The post surgical pain should be easily controlled with the prescribed medications and cool compresses. Any intense pain, unrelenting nausea/vomiting, or changes in color perception should warrant immediate notification to your surgeon.
Intense activities or anything that will raise your heart rate or blood pressure should be avoided for the first two weeks after eyelid surgery. The purpose of these restrictions is to minimize the risk of bleeding.
Yes, an upper eyelid blepharoplasty can be performed awake, in-office under local anesthesia. A lower blepharoplasty is performed under general anesthesia at the outpatient surgery center. Combined upper and lower eyelid surgery would be performed at the same time under general anesthesia at the surgery center.
Although eyelid surgery can turn back the hands of time, giving you a more youthful, rested appearance, it cannot stop the natural aging process. Without the ability to stop time, gravity and genetics, your face and eyelids will continue to age normally after surgery. The best ways to protect the investment you’ve made in yourself is to avoid UV exposure, abstain from any nicotine products, maintain a healthy diet and routine exercise, and use our recommended medical-grade, prescription skin care.
Yes, upper and lower blepharoplasty are suitable procedures for men and women. Male patients are more likely to have a heavy brow contributing to eyelid excess or sagging.
