Dermal fillers are synthetic hyaluronic acid products that closely mimic our tissues’ naturally occurring hyaluronic acid. The purpose of a filler is to add volume to the desired treatment area. Dermal fillers today provide a wide range of cohesiveness that can be used to improve specific areas. Typically, softer, less cohesive fillers are placed more superficially in the face like lips, laugh lines, and fine lines around the mouth. Denser, more cohesive fillers typically provide more structure. These fillers are often used in the deep face for enhancement of cheeks, jawline, or chin. Depending on the product used, dermal fillers can last 6 to 18+ months.
Botox is a brand name of an injectable neurotoxin. Other brands or types of neurotoxin include Dysport, Daxxify, Xeomin, and Jeuveau. All these toxins are aimed to stop or decrease muscle contraction. Repetitive muscle contraction in areas like the forehead, crow’s feet, and between the eyebrows leads to wrinkles visible at rest and with expression. Precise injection of the toxin into desired treatment areas can prevent muscle activation. In turn, this leads to a reduction in static and dynamic wrinkles. The results from neurotoxin treatment are temporary and dose-dependent. The ideal duration of effect is 3 – 4 months. The dosing of the product should be adjusted until the patient sees results lasting 3 – 4 months. FDA recommended dosing to get 3 – 4 months of anti-wrinkle effects is 64 botox-equivalent-units for the upper face.
HA fillers and neurotoxin treatments are synergistic. Often, both modalities are required to achieve your desired results. For example, let’s say you have a vertical line between your eyebrows at rest. The crease becomes more prominent when you’re angry or concerned. The neurotoxin would stop the eyebrow muscles from worsening the crease. Once that motion has been stopped, the dermal filler can be used to soften, smooth, and fill the crease. Placement of filler without stopping the muscle contraction with the toxin would be ineffective and unsatisfying.
Dermal filler comprises synthetic hyaluronic acid strands crosslinked and suspended with lidocaine. The implantable gel is dissolvable and safe. The amount of HA is standardized in each syringe but varies between the different brands and types of filler. For example, every syringe of Juvederm Ultra has 24 mg of HA per mL/cc, while Restylane has 20 mg/mL. Fillers differ in their degree of cohesivity, firmness, and affinity for water (which translates to a plumping effect).
Dermal fillers can be a great tool/treatment/option/etc for non-surgical facial rejuvenation. Certain dermal fillers can be placed to improve skin hydration. The appropriate filler can be selected according to your treatment goals. For example, if your cheeks feel flat or dull, denser, more cohesive fillers with a higher G’ can be used to restore structure and support. These fillers can give a more youthful lift to the midface with ~18 months of longevity. The volume in one syringe (1 cc) of filler is equivalent to a small blueberry. Most patients seeking non-surgical facial rejuvenation require 4 or more cc’s of filler to reach a desirable outcome. A sample treatment plan may include:
- 2 syringes for cheeks (1 cc per side).
- 1 syringe for jawline/jowls/marionette lines.
- 1 syringe for nasolabial folds (0.5 cc per side).
Although this may seem like a large quantity of filler, imagine 4 small blueberries of volume.
This is an important factor to consider upfront before your consultation and treatment. Sometimes, we recommend deferring your treatment until you are comfortable with the recommended volume. Otherwise, only 1 syringe may feel well-spent when you don’t see the desired results. Your customized treatment plan will be developed at your consultation, guided by your input and desired goals.
The areas most often treated with dermal fillers include lips, nasolabial folds, marionette lines, cheeks, jawline, chin, and fine lines around the mouth (sometimes called lip lines or smoker’s lines).
Other areas that are less commonly treated include:

These annual treatments often require much less filler compared to your initial rejuvenation treatment.
Dermal fillers are delivered to the desired treatment area through a needle or cannula. Sensitive areas like the lips are numbed with topical anesthetic before treatment. During your treatment, we utilize distraction techniques with vibration and acupressure. Pro-Nox is also available to keep you comfortable during the treatment.
Placement of dermal filler requires the use of a needle or cannula. Any break in the skin barrier can cause a bruise. No makeup should be applied immediately after your treatment.
